Weekend Palliative Home Care RN
Beaumont's Care & Treatment for Lifelong ConditionsSM team is seeking an RN to work weekends only. Shifts and days can vary.
The Care & Treatment for Lifelong ConditionsSM (TLC) is Beaumont's specialty palliative home care team providing intermittent, skilled care with a palliative philosophy to patients with advanced, complex illnesses. The RN Care Managers who are part of the Care & TLC team learn how to manage a patient's home care episode as part of an interdisciplinary unit, including nurses, social work, therapists and a Medical Director. The RN Care Manager meets biweekly with the team to collaborate on how to provide palliative aspects of care to current patients. In addition to the routine skilled home care the Care & TLC RNCM addresses pain and symptom management from a team approach, helps patients and families with complex decision making, advance care planning, and transitions of care, such as hospice. The nursing care is provided to all age groups of the population served, through conscious application of theoretical knowledge, skill, and coordination with members of the health care team.
RN Care Managers may function as any or all of the following: diagnostician, caregiver, counselor, planner, supervisor, evaluator, advocate, coordinator, record keeper or consultant. The RN Care Manager uses specific age and cultural-related physical, intellectual, psychosocial, and developmental attributes in the assessment, planning, implementation, and evaluation of patient care.
Requirements:
Must be able to perform basic life-support measures such as CPR and have a current BLS certification through the American Heart Association
Licensed to practice as a Registered Nurse in the State of Michigan
Completion of a Nursing program through an accredited school; BSN preferred
Must possess strong interpersonal skills and work well within an interdisciplinary team
Hospice nurses must have 2 years hospice experience, and be willing to learn all aspects of home care
Home care nurses must have a minimum of one-year experience in home care nursing. Nurses who have experience in both hospice and home care are preferred
